First Visit

First Visit for Family Practice and Primary Care:

The Community Check Up Center of South Harrisburg has heard you! We understand that getting an appointment when you are sick is very important to you. We know that we have had a lot of difficulty meeting your need for same day appointments. Beginning January 1st, we have moved to open access scheduling. This means that every day we have a provider in the office we will have up to 10 appointments available for same day appointments when we open in the morning. Please call us first thing in the morning when you need an appointment and chances are you will get seen that day! . If you call later in the day you may be offered an appointment the next day. Open access scheduling does not guarantee you an appointment today, but it does make it easier to get an appointment when you really need one.

When making your first appointment, you will be sent a message 3 days prior to your appointment to your email and/or cell phone. This message is VERY important to open and follow the directions. We have implemented a new pre-registration process. This message will prompt you to update all of your information and to answer registration questions all before coming into the office. This pre-registration will reduce the amount of time of your visit to our clinic.

When coming to the Community Check-Up Center for your first visit with your child, be sure to bring the following items:

  • Insurance Card

  • Picture ID

  • Social Security Number

  • Name of your pharmacy

When you check in with the Receptionist, you will be asked questions such as your address, phone number, email address, etc. You will also be asked about the size of your family and family income. Because we receive some government funds, we are required to get this information from all patients.

After completing the information with the Receptionist, you will be asked to take a seat in the Waiting Room until it is your child’s turn to be seen. You will be called back by a Medical Assistant who will take your child’s measurements, blood pressure, temperature, and ask about the reason for your visit. Then the doctor will see you. You will also have an opportunity to ask any questions you may have.

Because we are a non-profit organization that depends on grants and donations, we encourage our patients to make a donation when possible.

First Visit for Family Planning:

When coming to the Community Check-Up Center for your first visit, be sure to bring the following items:

  • Insurance Card

  • Picture ID

  • Social Security Number

  • Name of your pharmacy

  • List of medications and dosage

  • Date of your last PAP

  • Medical Records (if previously seen by another provider)

It is also helpful if you can provide the date of your last menstrual period.

When you check in with the Receptionist, you will be asked questions such as your address, phone number, email address, etc. You will also be asked about the size of your family and family income. Because we receive some government funds, we are required to get this information from all patients.

After completing the information with the Receptionist, you will be asked to take a seat in the Waiting Room until it is your turn to be seen. You will be called back by a Medical Assistant who will check your weight, blood pressure and ask for the reason for your visit. If indicated, a urine sample may be requested. Then the Nurse Practitioner will see you. You will also have an opportunity to ask any questions you may have.
