A full-service Family Practice for Men, Women, and Children

Our Providers see patients from birth to death for well-child checks, physicals, and visits for acute situations. We have approximately 4,000 visits a year. Referrals are made for services not provided at The Community Check-Up Center.

This includes:

• Management of chronic illnesses

• Preventative health screenings

• Immunizations
